Home Assessments
For Safe Discharge

After completing your wellness journey at Burton Health Care Center, you want to be sure the independence you regained while you were with us will continue to serve you when you return home. That’s why we offer optional home assessments. After completing a detailed assessment of your current abilities, the daily tasks you most struggle with, and your continuing goals, our highly trained recovery experts will visit your home. Once there, we’ll conduct an audit to determine if additional safety features, such as grab bars, ramps, and emergency call systems, are necessary, and whether we can provide additional training and support to make your transition smoother.
